ECH is expanding and our Care Coordination team are seeking a Care Coordinator Support Officer (CCSO) to join our team in the west region, based at Henley Beach.

**The role**

As CCSO you will provide administrative and co-ordination support to the Care Coordination team with a continual focus on improving process and delivering exceptional customer service.

The CCSO role will also include managing a case load of clients. You will work closely with our clients, their families, carers, and advocates to ensure all clients have a Care Plan that reflects their individual care and service needs to enable them to have the best life possible as they age in their own home.

You will be provided with full training and support including all equipment that is required to succeed in this role.

**What you’ll be doing**

As the primary point of contact, ensure exemplary customer service is delivered to both external and internal stakeholders by:

- Providing administration support to the Care Coordination team
- Assisting with client intake, assessment, and reviews
- Guiding new and potential clients to easily access services
- Establishing service and equipment requests
- Working collaboratively with the team to assist with the coordination of service for our clients

**You will need**
- COVID-19 vaccination (including booster if eligible)
- Drivers Licence

**It’d also be helpful if you had**:

- A current Police Check
- Diploma in Community Service Coordination or Certificate 3 in Aged Care or equivalent
- Experience and/or knowledge of Home Care Packages and Commonwealth Home Support Program
