We are seeking a compassionate and reliable **Support Worker** to provide **direct support** for a **43-year-old male** participant diagnosed with **Schizophrenia, developmental delay/adjustment disorder,** and a **history of poly-substance abuse**.

The participant is currently an **inpatient at Hornsby Adult Mental Health Unit** and has been granted **escorted leave**. This role involves providing **3-4 hours of direct community support** during escorted leave, ensuring safety, engagement, and positive social interaction.

**Key Responsibilities**:

- Provide **escorted community access and supervision** during approved leave hours.
- Support the participant with **daily activities, social engagement, and structured routines**.
- Promote independence, safety, and dignity while maintaining professional boundaries.
- Monitor and report on behaviour, mood, and engagement levels.
- Maintain accurate and timely **shift notes and incident reports**.
- Communicate with team leaders, clinicians, and relevant stakeholders to ensure consistent care.

**Essential Criteria**:

- Minimum **6-12 months of experience** supporting participants with **mental health or complex behavioural needs**.
- Certificate III or IV in **Individual Support, Mental Health, Community Services, or equivalent**.
- **Current NDIS Worker Screening Check** and **Working With Children Check (WWCC)**.
- **First Aid and CPR certification**.
- Strong interpersonal and de-escalation skills.
- Ability to work independently with professionalism and empathy.

**Desirable**:

- Experience working with participants with a dual diagnosis (mental health and substance use).
- Familiarity with trauma-informed and recovery-oriented approaches.

**Shift Details**:

- **Duration**: 3-4 hours per shift
- **Setting**: Escorted leave from Hornsby Adult Mental Health Unit
- **Start Date**: ASAP

**Job Type**: Casual

Pay: $32.56 - $45.00 per hour
