Why this role matters

The Salvation Army Australia’s Homelessness Stream provides high quality, person-centred services to people at risk, and or experiencing homelessness. We support all cohorts in case management, outreach support, financial assistance, connection and referral to other specialist services.

As a Case Manager you will provide case management support, in both a community and/or residential based setting, over the phone or in person. This support will assist people to achieve housing stability, experience improved social inclusion and achieve a level of self-mastery that will enable them to sustain tenancy and lead their best lives.

This is a permanent full-time role based in Stafford.

The role you could play
- Establish effective relationships with clients that encourage self-determination and participation
- Provide case management support to individuals in accordance with TSA case management models and standards of practice
- Complete thorough assessments, inclusive of needs, risks and vulnerabilities
- Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients, as well as barriers to achieving those
- Assist clients to access support services and establish community supports, and where needed are assisted through advocacy with service providers
- Regularly monitor progress and outcomes of individual case plans and where needed action is taken to ensure the plan continues to meet client needs or goals

What you will bring
- Tertiary qualification in Community Services, Social Work or related field. A degree level qualification is preferred
- Relevant experience in a social service environment including in client assessment and case management
- A Working with Children Check is required
- Driver’s Licence
- A willingness to undergo a National Police Check
