An-acc Coordinator

We have a vacancy for a full-time AN-ACC Coordinator in our organisation. As AN-ACC Coordinator, you will be responsible for providing consistent, appropriate and coordinated specialist expertise and advice in relation to Care Assessment associated with AN-ACC requirements of residents throughout the organisation.
Requirements of the role:
- Be able to identify AN-ACC rates in correspondence to their care needs required by individual resident based on care factors.
- Conduct AN-ACC Assessments while working closely with Senior Management.
- Confirm Assessments, care strategies and ongoing documentation are up to date and provide the evidence to support AN-ACC funding to receive best outcomes.
- Work collaboratively and constructively within a team, i.e., regular meetings to validate AN-ACC classifications.
- Consultative approach with external assessors visiting for the purpose of AN-ACC assessment.
- Provide expert advice and support to Residential Aged Care staff around funding evidence requirements, work along the teams assisting in the resident assessment process.
- Review processes and documentation, and conduct AN-ACC audits, to identify gaps or trends and then implement staff education in line with the best practice guidelines, organisational policies, and relevant legislation.
- Coordinate or deliver AN-ACC training to management, nursing and care employees
- You will be committed to track funding for each AN-ACC resident.
- Enter AN-ACC data into computer system and monitor expiry dates, annual reviews and new admissions.
- Regularly conduct reviews of the AN-ACC ongoing claims, analyse AN-ACC data, and generate reports.
About you:
- RN with Current Nurse Registration with AHPRA.
- Demonstrated understanding and knowledge regarding the AN-ACC framework.
- Previous experience and knowledge of AN-ACC systems and have worked in a similar role.
- Experience in care planning, review, and funding evaluation.
- Proven experience in achieving AN-ACC set key performance indicators.
- Knowledge of the aged care industry compliance and regulations.
- NDIS Worker Screening Check within the last 5 years or current Police Check while waiting for NDIS Worker Screening Check.
- Current Influenza and COVID-19 vaccinations.
