We are seeking qualified and committed **Sub-Acute Referral Liaison.** This role is pivotal in being the first point of contact for referring partners, establishing and maintaining a central intake and referral system, and managing datasets vital for funding partnerships and our mental health services.

We deliver services across the ACT covering children, families, young people and seniors including housing and mental health services.

**Our Offer**:

- Fixed-term full-time appointment (38 hours per week) until 31 December 2025.
- Salary of SCS Level 5 $99,643.17 - $104,152.19 p.a. plus 11.5% Superannuation.
- Salary packaging option up to $15,899 per Fringe Benefits Tax year, with an additional Meals and Entertainment package available up to $2,650 per Fringe Benefits Tax year.
- 15 days paid personal leave per annum (pro rata) for full & part-time employees.

**Experience & Qualifications**:

- Minimum certificate IV in mental health or community/health-related field, supplemented by a minimum of two years’ experience in coordinating referrals, intake and caseload management.
- Proven ability to evaluate and report on referral pathways, contributing to program objectives.
- Demonstrated ability to establish and maintain key relationships within the sector.
- Strong oral and written communication skills for interacting with diverse individuals and groups.
- Capacity to handle sensitive and confidential enquiries with discretion.
- Proven ability to build rapport and maintain sound relationships with participants, carers, and families based on mutual respect and trust.
- Demonstrated understanding of professional boundaries, privacy, and confidentiality.
- Demonstrated ability to work independently with mínimal supervision.
