**About the role**:
You will join a dynamic Restorative Care Team to deliver high-quality Exercise Physiology services to HammondCare clients in the Community. This is an exciting role with a broad caseload. Whilst you’ll have the autonomy to structure your own day to meet key deliverables, you will also be supported by an experienced MDT team.
Together you will deliver client-centred, evidence-based care in an environment that promotes continuous quality improvement consistent with the values and ethics of HammondCare and Exercise & Sports Science Australia (ESSA).

**What will your key responsibilities look like?**:

- Provision of a high-quality Exercise Physiology service to HammondCare HCP (Home Care Package) and CHSP (Commonwealth Home Support Program) clients including individual 1:1 home visits and group activities
- Assess and provide intervention to clients in their home, carry out in-home rehabilitation programs to improve functional independence and quality of life
- Collaborate closely with our skilled MDT team to assess clients’ functional care needs, set care goals, and provide treatment and interventions to deliver best possible client outcomes
- Participate in quality improvement projects and continuing professional development
- Provide clinical supervision for students, if appropriate

**About you**:

- Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action
- A tertiary qualification in Exercise Physiology & Exercise Sports Science Australia (ESSA) Accreditation
- Practical experience and/or sound knowledge in a Rehabilitation and/or Aged Care setting, across inpatient/outpatient and/or community settings
- A current driver’s license and safe driving record, with a willingness to travel between HammondCare’s sites and services
- Effective communication and interpersonal skills (written and verbal)
- Demonstrated organisation and time management skills
- Demonstrated ability to work independently and as part of a multidisciplinary team
