In your role you will manage client intake for aged care recipients eligible for Home Care Packages, this will include comprehensive assessment, coordinating delivery and brokerage services within the package budget and communicating with family and other service providers as required.

**'A week in the life'**
- Provides client assessment and development of interim care plan to support the client at home
- Provides advanced holistic care skills within a Case Management framework
- Demonstrates an understanding of the target population and their needs, from care coordination to complex case management
- Capable of managing a diverse and challenging intake caseload including clients with complex health &/or wellbeing needs on high level packages.
- Demonstrated expertise in budget management within a CDC framework
- Ability to work within the required legal framework underpinning the Home Care Packages Programme
- Liaises with packaged care team members to ensure care recipient needs are met in line with objectives and timelines

**Selection Criteria - Qualifications and Experience**
- Tertiary qualifications or certified training relevant to Aged Care, Community Services, Nursing or Allied Health
- Demonstrated understanding and knowledge of clients' needs aligned to the various funding programs (e.g. Home Care Packages, Commonwealth Home Support Program, Department of Veterans Affairs, Veterans Home Care)
- Highly developed assessment skills and the ability to creatively problem solve care situations
- Ability to manage a budget effectively
- Case management and community care experience
- Excellent communication and interpersonal skills
- Proven ability to contribute to and, work as part of a dynamic team in addition to operating independently
