**Your Qualifications**

We are looking for a qualified Aboriginal and Torres Strait Islander Health Professional that:

- **Aboriginal Health Worker**: Holds or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care. Aboriginal Health Workers willing to obtain Certificate IV in Aboriginal Primary Health Care (Practice) will be supported to transition to an Aboriginal Health Practitioner position upon completion and registration with AHPRA.
- **Aboriginal Health Practitioner**: Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and is registered with the Australian Health Practitioner Regulation Agency (AHPRA).
- Is eligible to drive in NSW and willingness and ability to travel as required.

**The Role**

There is a focus on prevention and management of chronic disease for Aboriginal clients and supporting them on their journey throughout custody back into community, particularly those with chronic or complex health conditions.

You will work collaboratively with nursing staff, Integrated Care Service, health professionals, Corrective Services NSW, Aboriginal Community Controlled Health Organisations and other relevant health and community groups to ensure the delivery of seamless, timely and effective health care to Aboriginal clients.

**A Transition to Practice Opportunity for Aboriginal Health Workers**

As a Transition to Practice position, we give Aboriginal Health Workers the opportunity to work while they study and meet the requirements for registration with the Australian Health Practitioner Regulation Agency (AHPRA) as an Aboriginal Health Practitioner. Through the transition, there is an expectation that clinical activities will be included and applied to daily operations as per scope allowances.

This position also allows direct appointment of existing Aboriginal Health Practitioners into the role.

Junee Correctional Centre is a mixed-security correctional centre for male offenders, situated just a few minutes from the town centre.
