**Position Title**: NDIS Support Coordinator
**Location**: Bendigo & Surrounds
**Reports To**: Directors of My Village Carers
**Employment Type**: Part time increasing to full time
**Salary**: As per SHADS award
**Position Overview**:
The NDIS Support Coordinator plays a vital role in assisting participants of the National Disability Insurance Scheme (NDIS) to navigate and access services, supports and resources in the community. This position involves working closely with participants to understand their individual needs, develop support plans, coordinate service delivery, and ensure that NDIS goals are met in a timely and effective manner.
The Support Coordinator also plays a key role in ensuring that participants are empowered to take control of their own NDIS plan and support system, while ensuring compliance with NDIS standards and policies.
**Key Responsibilities**:
**Support Coordination and Case Management**
- Work with NDIS participants to assess and understand their needs, preferences and goals.
- Develop and implement individualized support plans, in collaboration with participants and their families or carers.
- Provide ongoing coordination and monitoring of services and supports to ensure goals are being met and participants are satisfied with services.
- Maintain a comprehensive understanding of the NDIS funding process and ensure that the participant's budget is being managed appropriately.
- Ensure services are delivered on time, and resolve any issues or barriers to service delivery in a timely manner.
**Liaison and Stakeholder Management**:
- Build and maintain positive working relationships with NDIS participants, service providers, and other key stakeholders.
- Advocate on behalf of participants to ensure they receive the best services available, including identifying and recommending suitable providers.
- Collaborate with healthcare professionals, therapists, and other service agencies to ensure the holistic support of participants.
- Ensure all services provided are aligned with NDIS plan goals, standards, and guidelines.
**Monitoring and Reporting**:
- Monitor and track the progress of NDIS plans, ensuring timely reviews and amendments as required.
- Maintain accurate and up-to-date case notes, reports, and documentation in line with organizational policies and NDIS requirements.
- Provide regular updates and progress reports to participants, families, and the NDIS where appropriate.
**Education and Empowerment**:
- Educate participants and their families on how to navigate the NDIS system and make informed choices about their supports and services.
- Empower participants to become confident and independent in managing their NDIS plans and connecting with the services they need.
**Compliance and Quality Assurance**:
- Ensure all work complies with the NDIS Code of Conduct, relevant legislation, and organizational policies.
- Contribute to continuous improvement efforts by providing feedback and participating in training and development opportunities.
**Key Selection Criteria**:
**Qualifications and Experience**:
- Tertiary qualifications in Social Work, Disability, Community Services, or a related field are highly desirable.
- Experience in a similar role or working in the disability or community services sector is preferred.
- Knowledge of the NDIS and the disability sector, including an understanding of NDIS funding, policies, and processes.
**Skills and Abilities**:
- Strong communication and interpersonal skills, with the ability to build positive relationships with participants, families, and service providers.
- Excellent organizational skills and the ability to manage multiple cases simultaneously.
- Ability to problem-solve and find solutions for participants facing barriers in accessing services.
- A person-centered approach and commitment to empowering individuals with disabilities.
- Familiarity with case management systems and documentation practices.

- Strong attention to detail, with the ability to manage deadlines and work under pressure.

**Other Requirements**:
- Current Working with Children Check and National Police Check (or willingness to obtain).
- A valid driver’s license and access to a reliable vehicle (if required).
- Willingness to travel to meet participants and attend relevant meetings.
