**The Opportunity**:
We have an exciting opportunity for a Home Care Employee to come join our vibrant, dynamic and driven team within the Community and Home Care Services department located at Woongarrah NSW.
The role of the Home Care Employee is to provide a high level of quality individualised care to Home Care Consumers while maintaining commitment to respecting the values, preferences, culture and spiritual beliefs of the consumer and their representatives.
This role is to cover a period of leave for up to 12 months.

**Role & Responsibilities**:
Being responsive to consumers and families’ usual routine and standards, including consulting with consumers to determine the schedule and method of delivery of home, lifestyle and care services outlined in the care plan.
Working as a collaborative team member using open, transparent communication with the consumer, their family and other Anglican Care employees.
Delivering care in a professional, person centred and aged appropriate manner, respecting personal choice and autonomy as a customer of the service.
Follow individualised care and support plans promoting a focus on wellness, enablement and where appropriate timely and sensitive palliative care.
Relating warmly to all consumers, respecting their dignity and self esteem and maintain confidentiality and cultural sensitivity
Maintaining a safe and welcoming environment for consumers attending any of our Anglican Care community centres
About you?
Certificate III in Individual Support or equivalent
Proven ability to deliver person centre care that respects the dignity and worth of the individual.
Current Senior First Aid Certificate (ongoing requirement)
Current driver’s licence with registered, comprehensively insured and reliable motor vehicle.
An understanding of safe work practices, ideally proven experience in the use of appropriate manual handling techniques.
Able to work effectively as an individual and as part of a team
National Police Check (Newcastle Anglican will consider the relevance of any reported outcomes)
