This vacancy is maternity leave relief for an initial period of up to (54 weeks) for (up to 38 hours per week); the hours and period of employment may change according to the maternity leave requirements. There is also the possibility of an extension at same / reduced hours.

**About the role**

Ballina Community Health is seeking a Social Worker to join the Transitional Aged Care Program team, to provide comprehensive and appropriate Social Work assessment and intervention as an integral part of the multidisciplinary health care team for patients/clients of the Northern NSW Local Health District (NNSWLHD).
