**ABOUT THE ROLE**:

- We are seeking a dynamic Diabetes Educator to join our team of specialist nurses and allied health staff.
- Your role would be to monitor and coordinate client’s treatment plans, educate them about their condition, connect them with health care providers in the community, and evaluate their progress. This role often acts as a liaison between medical staff and clients to ensure that the best possible care is received and the client achieves their goals.
- Health Independence Program based at Werribee Mercy Hospital
- 0.42 EFT, permanent position
- Working Monday - Friday office hours, weekends and public holidays off
- Salary is in line with the Enterprise Agreement (hourly base rate $55.48)

**ABOUT YOU**:

- Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA), holding a current practicing certificate with current AHPRA registration
- Graduate Certificate in Diabetes Education
- Credentialed Diabetes Educator (CDE) with the Australian Diabetes Educators Association (ADEA) OR working towards ADEA credentialing
- Current Full Victorian (or equivalent) Drivers Licence
- Demonstrated clinical competency in the management of people living with Diabetes
- Experience in sub-acute/ community setting

**Desirable**:

- Experience and knowledge in the management of people living complex, chronic ,physical and psychosocial care needs
