BHN/Better Health Network is seeking a Care Advisor to join our friendly and experienced team and provide an exceptional service to our home care package clients. The purpose of this position is to provide high quality Care Coordination support to older people who have been allocated a Home Care Package (HCP), enabling them to live independently their homes as they age and improving quality of life. This will include using a Consumer Directed approach to implementing and monitoring care plans and delivery and brokerage of services within the package budget. **Duties and Responsibilities**:

- Managing incoming referrals and signing new clients.
- Meeting with potential clients and their families to navigate through the home care package process and understand their care needs.
- Developing a holistic care plan and budget in consultation with clients and their representatives to remain safe and independent in their home.
- Completing necessary documentation including client service agreement during new sign-ups.
- Reviewing care plans and budgets annually and as required.
- Being the first point of contact for all client queries and concerns.
- Ensuring the service is delivered to clients in a timely manner with high quality care as per the Aged Care Standards.
- Reporting any incidents and feedback, including SIRS as per organisational & legislative requirements.
- Liaising with local ACAS/ internal & external stakeholders/ GPs etc., as required to provide uninterrupted support to our clients.

**Essential attributes**:

- Have a minimum experience of 2 years as a case manager ideally in aged care or NDIS (essential).
- Hold a Nursing, Social Work or Allied Health background (desirable but not essential).
- Hold required registration for professional body (If applicable)
- Experience in using MS office, SharePoint and E-tools (desirable)
- Have a strong understanding and knowledge about aged care standards in community aged care settings.
- Ability to work independently and make the right decision to support HCP clients.
- Attention to detail.
- Good customer service skills.
- Current unrestricted Australian Driver’s License.
