**Who are we**
VIA Care is a support provider focused on working with a diverse range of participants. We support those ranging from children to older adults and from a range of cultural and linguistic backgrounds.
VIA Care is dedicated to bringing a trauma informed and family inclusive practice approach to disability support and recognises that disability supports must work collaboratively with other formal and informal supports.
**The role**
We are looking for an experienced Support Coordinator to join our diverse and growing support coordination team. As a Support Coordinator you will:
- Help NDIS Participants to build upon their strengths and overcome their barriers with the vision to improve their quality of life.
- Work collaboratively to empower each Participant to be the beating heart of their care team.
- Assist with linkages to service providers who are like minded in their focus on the benefit of the Participant.
- Build the capacity of each Participant to take achieve their goals and feel that their support network is helping them.
- Support staff to share in the benefit and success of VIA Care and all our Participants.
VIA Care welcomes applicants from people with a lived/living experience of disability and / mental health difficulties, people who identify as Aboriginal and / or Torres Strait Islander, people from the CALD and / or LGBTQIA+ community.
**What you will gain**
Training and development are at the core of VIA Care. We will seek to work with you to understand what opportunities for growth you would benefit from and how we can best help align the supports you’re provided. We offer regular 1:1 supervision and monthly peer group meetings to share and grow together.
**Eligibility criteria**
- Tertiary qualification and experience related to disability, allied health, social work or case management.
- Understanding of the NDIS in purpose and implementation.
- Commitment to delivering a tailored service that meets the aspirations of voluntary clients and delivers improved quality of life across all domains.
- Demonstrated understanding of best practice in disability or an allied health setting.
- Ability to work independently and self-motivated to accomplish objectives for participants and organisation KPI’s.
**Job Types**: Full-time, Part-time
Part-time hours: 30.4-38 per week
**Salary**: $65,000.00 - $85,000.00 per year
