Merri-bek is a great place to live, work and play. **About the role**: We have an exciting part-time temporary opportunity for an Age Well Planner/ Care finder to join the team. Split role between both Age Well and Care Finder, be a part of the development of the New Merri-bek Navigation Service as well as be apart of the Care Finder Program which operates across council boundaries with Darebin and Moonee Valley. The Age Well Planner will be responsible for supporting Merri-bek residents to be able to access the services they need, by increasing their knowledge of My Aged Care, developing an Age Well Guide, providing regular "Ask us Anything" sessions in the community as well as one on one support via the phone or in person and at our local events and festivals. As well as regular evaluation and reporting of progress.
