**ABOUT THE ROLE**:

- The Dementia Advisory role is a full time opportunity where no two days are the same. Utilising your clinical and care management experience, you will work with clients and their representatives to develop individualised care plans that will provide specialised care to clients with dementia as well as providing support, education and advice to direct care staff caring for clients with dementia.

**ABOUT YOU**:

- To be successful, you will have extensive knowledge of dementia care and CHSP funding guidelines, demonstrated assessment and care planning skills and previous experience managing complex client’s needs within a community, aged or disability setting. A relevant qualification in Allied health or Nursing is essential.
