**Description**:
About the Role

As the Aboriginal Health Worker, you will contribute to where possible to the ATSICHS comprehensive health support services to clients with the aim to achieve improved health outcomes for all Aboriginal and Torres Strait Islander people. This includes effective: triage, prevention, education, support and recalls.

Duties
- Maintaining health records
- Acting as communicator and interpreter on behalf of clients and other health workers
- Taking part in case management and follow up, either independently or with other health care providers
- Providing health education to individual clients and health staff
- Providing cultural education to people outside the cultural community
- Providing life skills education, counselling and referral for crisis intervention in the community they serve
- Providing input into the planning, development, implementation and monitoring and evaluation of all health programs in the community.

**Skills and Experiences**:

COVID-19 Requirements
- A medical certificate signed by a medical practitioner stating you have received both doses of a COVID-19 vaccination
- Your immunisation history as listed on the Australian Immunisation Register (AIR) that clearly shows at least one dose of COVID-19 vaccination details
- Your exemption as listed on the Australian Immunisation Register (AIR) may be submitted.

Required

Certificate III in Aboriginal Primary Health Care (Clinical Practice)
Previous demonstrable experience in relevant Aboriginal health worker field.
Demonstrated ability to meet targets and performance outcomes.
Ability to support a continuous improvement model for achieving outcomes.
Decisive listening, influencing and persuasive skills.
Working with Children’s Card (Blue Card) or be eligible to apply

**Desirable, but not mandatory**:
Minimum two (2) years’ experience in a Primary Health setting.
Previous experience of working with Aboriginal and Torres Strait Islander clients.
Previous experience in a multidisciplinary health team.
Previous experience in chronic disease management.
