**About Us & The Role**

The Specialist Dementia Care Program (SDCP) is a federal initiative in partnership with the state and territory governments in supporting care of residents in approved residential aged care facilities, who provide specialised care to people living with Behavioural and Psychological symptoms of Dementia (BSPD). The aim of the program is to reduce /stabilise symptoms in order to assist people to transition back to mainstream residential aged care. Key Objectives of the SDCP are to:

- Provide support for people living with Behavioural and Psychological symptoms of Dementia (BPSD) who are currently unable to be supported in main stream residential aged care
- Provide person centred, best practice care for people experiencing severe BPSD
- Complement existing Australian Government funded dementia support services including Dementia behaviour Management services (DBMAS) and Service Behaviour Response team (SBRT).
- Generate evidence on best practice that can be adapted to support people with BPSD in main stream residential aged care

Hammond Care, Caulfield was successful in securing the operation of the SDCP for the South Eastern Primary Health Care Network and Monash Health is funded to provide specialist support to the Caulfield SDCP.

**About You**

The SDCP clinician role is to provide the specialist support and model best person-centred, evidence based clinical care. Working within a multidisciplinary team to assist in achieving a holistic, biopsychosocial assessment of the person, understanding the reasons they are experiencing BPSD and working to establish nonpharmacological strategies to support and improve their symptoms and experience. In addition to those stated in position description, technical skills/knowledge/ experience required includes:

- Ability to provide expert clinical consultancy
