**About this Program**

Wellways programs in the ACT offer Step Up, Step Down at Home outreach support to people with a mental illness living within the ACT. The Step Up, Step Down at home approach provides alternative options to hospital admission, early intervention and more options for support for people with a mental illness particularly following a presentation at the Emergency Department with a mental health concern and not being admitted, an unsuccessful referral to the ACCESS team, repeated presentations to the watch house and/ or increasing concerns from the GP about someone's mental health. Outreach support provides transitional support for those likely to enter the Step Up, Step Down service or returning to their usual community accommodation.

The programs aim to improve mental health outcomes for the target group and to provide opportunities to increase knowledge, skills and confidence to manage future crisis through increased awareness of and the capacity to access community resources and support networks as well as education and training and employment where appropriate.

Evidence of development of personal resources of participants is measured using valid recognised outcome measures. The service targets people over 16 years of age who experience mental illness and operates 52 weeks per year with service delivery hours matched to the needs of the target group.

**About the opportunity**

The Community Outreach Worker is responsible for providing direct support to participants within the framework of the organisations policy and procedures, all applicable legislative standards and any relevant accreditation standards and agreed SLA's, particularly around caseloads and hours of service. You will also contribute to the effective operation and on-going development of the program offer to ensure that the program reflects Wellways values, best evidence-based practice and demonstrates innovation.

**You will need**:

- Tertiary Qualifications in a social science or related discipline
- Current valid Driver's License and the ability to undertake travel for the role
- Current Working with Children Check and NDIS Workers Screening Check
- Right to Work within Australia
- Satisfactory National Police Records Check
- NDIS Worker Orientation Module Certificate
- Evidence of COVID vaccination with booster or valid medical exemption
