We deliver a range of primary health, social care and community-based support to meet local community needs. We have approximately 300 staff, with seven (7) locations across Ballarat and the Western Region.

**About the role**

As a Child Health Case Manager, you'll:
Build trusting relationships with children and families, helping them navigate challenges with kindness and empathy.
- Provide case management for a small caseload (8-10 families), developing tailored support plans that promote stability and wellbeing.
- Work alongside a multidisciplinary team, strengthening connections between allied health, paediatrics, and family support services.
- Be proactive and solution-focused, ensuring families receive the appropriate supports at the right time.
- Offer guidance and advocacy, empowering families to make informed decisions for their child’s health and development.
- Support the specialist paediatric clinic, collaborating across disciplines to improve outcomes for children.
- Support families to link in with NDIS.
