Community Mental Health Specialist

The Community Mental Health Specialist plays a vital role in providing high-quality support to individuals with mental health needs. This includes conducting comprehensive assessments, developing care plans, and implementing interventions to promote recovery.

Key responsibilities include:

  • Providing intensive support to clients post-discharge from hospitalization.
  • Conducting risk assessments, crisis planning, and care development for high-risk clients in the community.
  • Preparing case-managed clients for court proceedings and engaging in relevant legal processes.
  • Participating in discharge planning and supporting patients transitioning from hospital to community settings.
  • Providing education and support to carers regarding ongoing treatment and care.

Requirements:

  • Demonstrated commitment to mental health consumer recovery principles and their application.
  • A minimum of 5 years' experience in mental health, preferably recent community mental health experience or extensive clinical mental health experience.
  • Approved degree in Social Work, Occupational Therapy, Psychiatric Nursing, or Psychology, registered with APHRA.
  • Knowledge of community resources, including drug and alcohol services and accommodation services.
  • Knowledge of relevant legislation, mental health systems, and the criminal justice system.
  • Proven experience in conducting mental health and risk assessments.
