Key Responsibilities
  • Develop and implement individualized support plans to meet the unique needs of participants
  • Build and maintain strong relationships with participants, their families, and service providers
  • Coordinate and facilitate access to funded and community supports
  • Provide education and advocacy to participants and their families to empower them to make informed decisions
  • Collaborate with internal and external services to maximize participant outcomes
Requirements
  • Tertiary qualification and/or minimum two years' experience working in the Community Services sector
  • Well-developed communication and interpersonal skills
  • Ability to work autonomously and cooperatively as a member of a team
  • Strong organizational skills and attention to detail
  • Current Australian driver's license and satisfactory completion of safety screening
