Mental Health Community Outreach Coordinator

This position plays a vital role in the delivery of community-based mental health outreach, prioritizing cultural safety and empowerment.
Main Responsibilities:- Supporting participants with daily living skills, transportation arrangements, social engagement, and medication management
- Maintaining accurate records using digital systems and tools
- Fostering respectful relationships with participants, families, carers, and community stakeholders
- Completing risk assessments, support plans, and documentation in a timely and culturally sensitive manner
- A certificate-level qualification in Mental Health or Community Services, or equivalent experience
- Experience working in mental health and/or community settings, with an understanding of NDIS and block-funded programs
- Knowledge of Aboriginal cultural protocols, community dynamics, and trauma-informed practice
- Basic digital literacy (Microsoft Office and other systems)
