About the Role

We are seeking a highly skilled Occupational Therapist to join our team as a Community Support Specialist. As a key member of our multidisciplinary team, you will be responsible for managing a caseload of participants aged 65 and over, providing support to maintain their independence at home.

Key Responsibilities:

  • Prescribe home modifications and equipment to enhance participants' daily living skills
  • Develop personalized treatment plans based on assessments and client goals
  • Collaborate with other healthcare professionals to ensure coordinated and comprehensive care

Requirements:

  • Graduate degree in Occupational Therapy and current registration with AHPRA
  • Adaptive communication skills to engage with varied participants, stakeholders, and staff members
  • Demonstrated experience working within the NDIS and/or Aged Care Frameworks and billable targets
  • Demonstrated experience using a variety of assessment tools and completing complex reports
  • Driver's license, reliable vehicle, and comprehensive insurance
  • Current NDIS Worker Screening Check, First Aid Certificate, and COVID-19 Vaccination
