About the Role:

The Team Leader will play a critical role in ensuring the delivery of exceptional care to our residents. This involves supporting the Clinical Care Coordinator and Registered Nurse to supervise and coordinate care, while actively promoting the individual needs, wishes, and goals of each resident.

Key Responsibilities:

  • Supervise and coordinate care to residents across the home
  • Support the Clinical Care Coordinator and Registered Nurse to ensure high-quality care
  • Develop and implement care plans in collaboration with multidisciplinary teams

Requirements:

  • Relevant Cert 4 qualification & medication endorsed
  • High level of commitment, integrity, honesty, and trustworthiness
  • An effective listener with excellent communication skills
  • Ability to work independently and as part of a team
