We are seeking an experienced and passionate Aboriginal Health Worker to join our team at SA Health. As a key member of our health service, you will provide holistic support services to Aboriginal clients in the Gawler community.
Key Responsibilities- Provide culturally sensitive health care services to Aboriginal clients, working in a collaborative and participative framework to increase continuity of care across the health setting.
- Facilitate Well Health Checks and promote better outcomes for Aboriginal people through networking and maintaining collaborative partnerships between service providers, non-government agencies, and the community.
- Contribute to service development activities and evaluate service effectiveness, assisting with monitoring care plans to ensure appropriate and relevant care outcomes are achieved.
- Work closely with the community to develop culturally appropriate models of care for Aboriginal Health and extend Aboriginal community engagement.
- Aboriginal and/or Torres Strait Islander heritage is considered essential.
- Confirmation of Aboriginality may be requested.
- Relevant qualifications and experience in Aboriginal Health or a related field.
