Job Title:

Community Health Social Work Manager

Role Description:

The Community Health Social Work Manager plays a key role in the delivery of high-quality social work services to community-based clients.

Key Responsibilities:

  • Manage operational requirements and provide clinical leadership, supervision, and mentoring.
  • Coordinate and support clinicians across various community health centres providing social work services through outpatient clinics, telehealth, and home visits.
  • Support adult clients with complex psychosocial needs impacted by health issues, including those with chronic conditions, frail aged conditions, and complex medical and psychosocial conditions.
  • Comply with Canberra Health Services occupational assessment, screening, and vaccination policy, as well as current registration under the Working with Vulnerable People (Background Checking) Act 2011.
