
Advanced Healthcare Specialist for Elderly Care

Our organization is seeking a skilled Gerontological Care Professional to join our Aged Care Assessment Services (ACAS) team.
The successful candidate will conduct comprehensive assessments to identify and meet the complex needs of elderly clients, referring them to suitable aged care services as per the Aged Care Act.
- Establish strong relationships with clients, families, and multidisciplinary teams as required.
- Collaborate effectively with colleagues to foster a supportive and positive work culture.
Utilize the Integrated Assessment Tool (IAT) to conduct accurate aged care assessments, determining client needs and eligibility for home support services.
Regularly review and update support plans to cater to clients' evolving support requirements.
Deliver results-oriented performance within designated timeframes, working both independently and as part of a broader team.
Maintain currency of knowledge regarding aged care services, encompassing both funded and non-funded options.
Assist in implementing continuous improvement frameworks to regularly assess and enhance assessment processes and outcomes.
Key qualifications and skills include:
- A tertiary qualification in a health-related discipline directly related to healthcare, gerontology, or a related specialist area.
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
- An Australian driver's license and access to a registered, insured vehicle.
- A minimum of one year's demonstrated experience in Australia or overseas, delivering services in aged care settings and/or to elderly individuals, including those living with dementia.
- Willingness to undergo a police check.
