Job Overview

We are seeking a highly skilled Disability Case Manager to provide Advanced Support & Recovery services to our customers. This role involves providing dedicated support to customers who have sustained severe or catastrophic injuries, ensuring they receive the necessary benefits, treatment, and healthcare services.

Responsibilities
  • Proactively manage case files in a one-to-one case management environment, supporting customer rehabilitation.
  • Leverage recovery and disability case management knowledge to assess customer needs and develop appropriate goals, milestones, and timelines.
  • Collaborate with health care professionals, health care facilities, and community stakeholders to coordinate customer care.
Requirements
  • An undergraduate degree or diploma in a relevant field (Kinesiology, Occupational Therapy, Physical Therapy, Disability Management, etc.) and professional experience in disability management or vocational rehabilitation.
  • Several years of direct proactive recovery/disability case management experience, including collaboration with health care providers throughout the duration of customer rehabilitation.
  • Meet the provincially legislated requirements of the Criminal Records Review Act (CRRA).
