We are currently seeking a dedicated and passionate Dementia Care Adviser to contribute positively to the lives of our clients.
Position Overview:
Part-time (4 days) - Flexible travel required within designated regions.
As a Dementia Care Adviser, you will be instrumental in delivering high-quality, person-centered support, guidance, and advocacy for individuals living with dementia and their families. Your role will enhance client well-being and promote community education regarding dementia.
Key Responsibilities:
- Client-Centered Approach: Provide respectful and supportive care to clients and their caregivers, ensuring they are treated with empathy and dignity. Facilitate services that empower clients to live their lives according to their unique needs.
- Case Management: Oversee case management in accordance with organizational policies, ensuring clients receive appropriate support.
- Advocacy and Support: Offer evidence-based advice and advocacy during home visits or group sessions, assisting individuals with dementia and their family caregivers in navigating challenges.
- Community Education: Organize educational events and discussions to raise awareness about dementia, supporting families and local networks.
- Exceptional Customer Service: Promote a culture of outstanding customer service within the team, focusing on the needs and preferences of clients and their caregivers.
- Assessments and Care Planning: Conduct and review assessments and care plans promptly, ensuring services are effective and tailored to client needs.
- Financial Oversight: Manage services within allocated budgets, ensuring efficient financial practices.
- Administrative Duties: Handle all administrative responsibilities, including service confirmations, compliance, scheduling, and client records, with accuracy and timeliness.
- Stakeholder Engagement: Build and maintain positive relationships with internal and external stakeholders, ensuring effective communication and confidentiality.
Qualifications and Skills:
- Diploma in Community Services/Dementia Care or equivalent qualification is essential.
- Comprehensive understanding of dementia, its progression, and its effects on individuals and caregivers.
- Proven experience working with older adults, particularly those living with dementia and their families.
- Excellent communication skills, both written and verbal, with strong presentation capabilities.
- Proficient in MS Office and client management systems.
- Valid driver's license and access to a vehicle.
