Clinical Aged Care Professional

The role of the Clinical Aged Care Professional is to deliver high-quality care and support services to older adults in their homes or community settings.

Key Responsibilities:

  • Conduct comprehensive assessments to identify the needs of clients, including those living with dementia, and develop individualized care plans to meet their complex needs.
  • Work collaboratively with multidisciplinary teams, families, and caregivers to ensure a supportive environment that promotes the health and well-being of clients.
  • Use evidence-based tools and frameworks, such as the Integrated Assessment Tool (IAT), to determine client needs and eligibility for home support services.
  • Monitor and review care plans regularly to ensure they are up-to-date and reflect changing client needs.
  • Develop and maintain strong relationships with clients, families, and other stakeholders to provide ongoing support and advocacy.

Required Qualifications and Experience:

  • A health-related tertiary qualification, such as a Registered Nurse or Occupational Therapist degree.
  • Current unrestricted registration with relevant authorities.
  • An Australian Driver's license and access to a registered vehicle.
  • Minimum one year of experience delivering aged care services, including to people living with dementia.
