We are seeking a highly skilled and experienced Nurse Coordinator to join our Paediatric Gastroenterology and Hepatology team at ACT Health. As a key member of our team, you will be responsible for coordinating complex care services for children and young people with gastroenterology and hepatology conditions.
Key Responsibilities- Provide expert knowledge and skills in coordinating care services for children and young people with gastroenterology and hepatology conditions.
- Work collaboratively with Paediatric Gastroenterologists, other Paediatricians, and stakeholders from the Centenary Hospital for Women and Children and Sydney Children's Hospital Network to ensure seamless transfer of care and support.
- Provide education, training, and follow-up support for families in inpatient and outpatient settings.
- Monitor and manage medication and biologic regimes for patients.
- Registered Nurse Level 3 qualification.
- Experience in paediatric gastroenterology and hepatology nursing.
- Strong communication and interpersonal skills.
- Ability to work collaboratively as part of a team.
- High level of organisational and time management skills.
