About the Role

We are seeking a highly skilled Geriatric Care Coordinator Specialist to join our Health Independence Program at Latrobe Regional Hospital.

Job Summary

The successful candidate will be responsible for coordinating care for patients with chronic diseases, including cardiac and respiratory conditions. They will work closely with healthcare professionals, families, and carers to develop individualized care plans that promote effective management of patient needs.

Responsibilities
  • Triage inpatients, including those attending the emergency department, to assist with case management in partnership with GPs and other supports.
  • Participate in rehabilitation programs to support patients with chronic diseases.
  • Assess and manage clients with complex needs, working collaboratively with multidisciplinary teams.
  • Provide education and support to patients and their carers on disease management and self-care strategies.
Requirements

To be successful in this role, you will require:

  • A Registered Nurse qualification with experience in gerontology or palliative care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
