As a Customer Experience Coordinator, you will play a critical role in supporting customers to achieve their goals and maintain choice and control over their lives.

The successful candidate will have experience in case management or customer service, with strong communication and interpersonal skills. You will be responsible for working with customers to develop and implement individualised support plans, as well as providing education and guidance to help them navigate the home care system.

Key responsibilities will include:

  • Managing a caseload of Home Care Package customers and delivering high-quality care-management services
  • Working with customers to recognise existing and build new capabilities to support their self-management journey
  • Efficiently utilising available resources for customer education and driving rapid adoption of the Mable platform
  • Monitoring, reviewing and developing assessment and support plans in collaboration with customers and their representatives
