About the Role

We are seeking a highly skilled and experienced District Nurse to join our team at St Vincent's Hospital Melbourne. As a District Nurse, you will provide high-quality nursing services to clients in their homes, promoting independence and improving health outcomes.

Key Responsibilities:

  • Assess and plan care for clients with complex needs, working collaboratively with multidisciplinary teams to ensure seamless care delivery.
  • Provide education and support to clients and their carers, empowering them to manage their health and wellbeing.
  • Liaise with local service providers and health professionals to ensure coordinated care and optimal outcomes.

Requirements:

  • Minimum 4 years acute clinical nursing experience, with a strong background in community care.
  • Relevant clinical experience in community settings and/or across a range of acute medical/surgical services.
  • Demonstrated highly effective interpersonal skills, including problem-solving, conflict resolution, negotiation, and teaching.
  • Experience working with clients with multiple comorbidities and complex care needs.
  • Excellent organisational, coordination, and time management skills.
  • Current nursing registration with AHPRA and a current driver's licence.
  • Current National criminal history check and NDIS Worker Screening Check.
