Home-Based Nursing Opportunities

We are seeking a skilled and compassionate nurse to join our multidisciplinary team in Sydney, Australia.

  • The ideal candidate will have a strong interest in mental health and be passionate about providing holistic, person-centred care to older adults experiencing health challenges, chronic conditions, and complex needs within their homes and community settings.

Key Responsibilities:
  • Providing direct clinical nursing care to clients in their homes in line with best practice nursing procedures
  • Conducting clinical assessments, dressings, medication assessment and catheter changing
  • Developing a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevant
  • Providing safe, quality nursing practice to clients with specific needs and/or complex needs eg. Wound care, palliative care, education and support
  • Liaising with all parties involved in the care of the client to maintain holistic health care and wellbeing
  • Ensuring all documentation is accurate and completed in a professional and timely manner
  • Working in a safe manner to protect yourself, your colleagues and clients closely following all policies and procedures
Requirements:
  • A genuine passion for providing high-quality care to the elderly
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar
  • Minimum 3 years post-graduate nursing experience
  • Experience in clinical assessments
  • Initiative and the ability to work independently
  • Strong computer skills especially Microsoft Office products and ability to pick up new systems
  • NSW Driver's license and a reliable vehicle
