About this Role:
Our organization seeks a skilled professional to assume the role of Care Coordinator. This position involves collaborating closely with clients, care managers, and service coordinators to deliver high-quality services tailored to the unique needs of our clients.
Key Responsibilities:
- Engage in detailed intake processes and assessments to identify clients' care goals and develop personalized care plans
- Collaborate with care managers and service coordinators to secure appropriate service bookings that align with clients' identified goals
- Monitor clients' progress against identified care goals and conduct regular reviews and assessments as needed
- Manage client budgets to ensure compliance with legislative guidelines
Requirements:
- A minimum of two years' experience in care management within the disability, aged care, or broader community care sectors
- Proven ability to support individuals through the development of care goals and personalized care plans
- Current driver's license (QLD)
- Formal training in case management through a recognized course or program
