Job Summary

The Clinical Social Worker at Northwestern Medicine provides comprehensive psychosocial assessments and interventions to patients and their families, ensuring a smooth transition from hospital to home or other living arrangements. The ideal candidate will have a Master's degree in Social Work from a CSWE-accredited program and licensure in Illinois as a Licensed Social Worker (LSW).

Key Responsibilities:
- Perform thorough psychosocial assessments of patients and their families to identify needs and develop effective plans for discharge.
- Collaborate with multidisciplinary teams to ensure timely discharges and coordinate post-hospital care services.
- Provide crisis intervention and Protective Services for vulnerable populations, including the elderly and victims of abuse/neglect.
- Act as an advocate for patients requiring assistance to access necessary resources and services.

Required Skills and Qualifications:
- Masters Degree in Social Work from a school of Social Work accredited by CSWE.
- Licensure in Illinois as a Licensed Social Worker (LSW).
- Excellent interpersonal skills to build strong relationships with patients, families, and healthcare teams.
- Strong organizational and analytical skills to prioritize and manage a caseload efficiently.
