We are seeking a skilled and dedicated clinical pharmacist to join our team on a full-time, fixed-term basis. As a key member of the pharmacy team, you will be responsible for delivering high-quality patient care and contributing to the development of our clinical services.
Job Description:- Compile and document best possible medication history for hospital inpatients
- Reconcile and clinically review prescribed medications
- Manage supply of inpatient and discharge medications for hospital patients and some outpatients
- Check prepared medications prior to delivery to wards and patients
- Prepare discharge medication and discharge medication summaries
- Provide medication education and counselling for patients and their carers
- Work within a team of pharmacists, technicians and students providing supervision and assistance as required and contributing to improvement activities
- Liaise with external facilities and community healthcare providers to ensure continuity of care for individual patients
- Provide medication-related information, advice and support to the multidisciplinary team
- Paediatrics and adolescent health
- Oncology and haematology
- Coronary care and interventional cardiology
- Obstetrics and gynaecology
- Special care and neonatal intensive care
- Bachelor of Pharmacy
- APHR A registration to practice as a Pharmacist (without restrictions)
