About the Role

We are seeking a dedicated Homelessness Support Case Worker to join our team in Townsville, Queensland. As a key member of our Afterhours Outreach Program, you will provide mobile, outreach and centre-based case management support to individuals and families experiencing homelessness or at risk.

Your primary goal will be to help clients make long-term lifestyle changes, enabling them to successfully participate in community life, education, and employment. You will work from a strength-based and capacity-building framework to promote social inclusion, human rights, participation, and consumer choice.

Key Responsibilities
  • Conduct holistic assessments of client needs and develop personalized case plans
  • Provide direct support to clients with complex needs, including those from culturally and linguistically diverse backgrounds
  • Collaborate with staff members and clients to ensure effective service delivery

Requirements
  • Sound casework/case management experience, including assessment and planning
  • Demonstrated ability to work directly with clients at risk of homelessness who have complex needs
  • Well-developed interpersonal and written communication skills
  • Computer literacy, with proficiency in Microsoft Office Suite
  • Working with Children Check (Blue Card)
  • Australian drivers license
Salary and Benefits
