Care Manager Role in Aged Care

Located in Croydon, NSW, we are seeking a motivated and hands-on mental health expert to pursue a career opportunity with Catholic Healthcare. As our new Care Manager, you will act as the clinical lead to ensure delivery of quality clinical and person-first care for our residents.

About the Role:

  • Lead a team of clinical care staff to focus on a culture of continual improvement and quality
  • Ensure compliance with Aged Care Accreditation Standards
  • Deliver high-quality resident care by updating care plans, wound reviews, and medication oversight

Key Responsibilities:

  • Develop and implement strategies to improve resident outcomes and satisfaction
  • Maintain open communication with residents, families, and healthcare professionals
  • Collaborate with the Residential Manager to drive excellence in aged care services

Requirements:

  • AHPRA Registration
  • Demonstrated work experience as a Clinical Care Manager with a focus on mental health outcomes
  • Extensive knowledge of clinical functions in an Aged Care home
  • Superior communication skills, both written and verbal
  • Ability to be self-motivated and promote a positive work environment
  • Fully COVID vaccinated
  • Will to obtain an NDIS clearance and submit a Police and Bankruptcy & Insolvency check
