
Caregivers for Home Healthcare Services

We are seeking skilled home healthcare professionals to join our team in providing essential care to individuals at risk of hospitalization.
This position offers flexibility, allowing you to set your own availability while making a meaningful impact on client health outcomes.
- AHPRA Registration – Must be registered with the Australian Health Practitioner Regulation Agency.
- Relevant Qualifications – Nursing and/or Midwifery qualification required.
- Vaccination Compliance – Ability to provide evidence of vaccinations to meet QLD Health requirements.
- National Police Certificate – Current National Police Check required.
- First Aid and CPR Certificate – Valid and up-to-date certification required.
- Australian Driver's Licence & Reliable Transport – Ability to travel between client locations.
Key Responsibilities:
- Provide advanced nursing care in complex situations, supporting interdisciplinary teams while managing medications and treatments.
- Skilled in wound assessment, tracheostomy care, enteral feeding management, and patient monitoring.
- Assess patients with chronic conditions, offering guidance on treatment options, symptom management, and lifestyle modifications.
- Adapt care plans to address cognitive, behavioural, and functional changes while ensuring patient dignity and ethical considerations.
- Monitor, evaluate, and report patient data to assist with escalation, risk management, and continuous quality improvement.
- Deliver patient-centred care to support health, well-being, and quality of life.
