The successful candidate will be responsible for delivering high-quality patient-centered care as part of a multidisciplinary team, working collaboratively with experienced Nurses, ANPs, HCAs, and Practice Nurses.

  • Key Responsibilities:

  • Manage a structured workload with protected administrative time

  • Deliver patient care with up to 30 patient contacts per day

  • No home visits required

  • Participate in weekly clinical meetings
