Job Role:

As a Direct Support Professional, you will play a vital role in empowering aged, frail, or disabled individuals to comfortably remain in their own homes with the support they need. You will work closely with a team of dedicated healthcare professionals, contributing to a dynamic, multi-disciplinary team. This position offers a rewarding opportunity to make a meaningful difference in clients' lives while fostering their independence and well-being in a supportive community setting.

You will provide person-centered care that meets the unique needs of each individual, promoting their physical, emotional, and social well-being. Collaborating with other healthcare professionals ensures seamless care transitions and optimal outcomes for our clients.

Key Responsibilities:

  • Provide high-quality care to clients in their own homes
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Contribute to the development and implementation of personalized care plans
  • Monitor and report changes in client condition to the supervisor
  • Participate in professional development opportunities to enhance your skills and knowledge

Requirements:

  • AQF Certificate III relevant to the position and at least 560 hours satisfactory in-service experience
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Capacity to maintain confidentiality and handle sensitive information
  • Willingness to work flexibly, including evenings and weekends
