Care Specialist

This 12-month fixed-term Care Specialist role offers a unique chance to make a meaningful impact in the lives of carers across the Central Highlands, Central Grampians, and Wimmera regions. Working as part of a supportive team, you will provide person-centred support to carers, helping them set and achieve their goals.
Key Responsibilities- Conduct comprehensive assessments and care planning sessions with carers
- Coordinate and review support services to meet individual needs
- Deliver targeted education and advocacy programs for carers
- Foster strong relationships with stakeholders and community networks
- Promote and develop services that cater to carers' needs
- Maintain accurate and timely records of client interactions
- Tertiary qualification in Allied Health, Nursing, or Human Services
- In-depth knowledge of carers' support requirements and available services
- Strong assessment, planning, and coordination skills
- Proficiency in Microsoft Office and database management systems
- CURRENT UNRESTRICTED DRIVER'S LICENSE
