Clinical Pharmacist Opportunity in South East Queensland

Mater Misericordiae Ltd is seeking a highly motivated and experienced Clinical Pharmacist to join our team in South East Queensland.
This is a rotational position working across the pharmacy dispensary as well as inpatient wards of our campuses. You will work closely with hospital staff and patients to deliver a consistently high level of clinical pharmacy service by providing customers, patients, carers and other health professionals with support and information relating to medication and medication safety practices.
Key Responsibilities:- Compiling and documenting best possible medication history for hospital inpatients
- Reconciliation and clinical review of prescribed medications
- Managing supply of inpatient and discharge medications for hospital patients and some outpatients
- Preparation of discharge medication and discharge medication information
- Checking prepared medications prior to delivery to wards and patients
- Medication education and counselling for patients and their carers
- Liaison with external facilities and community health care providers to ensure continuity of care for individual patients
- Providing medication related information, advice and support to the multidisciplinary team
You will be working across public and private inpatient areas including general medical, surgical and obstetric wards as well as the inpatient dispensary which manages medication distribution and services procedural areas, Hospital in the Home and the Mater Infusion Clinic.
Depending on your experience and interest, opportunities exist to develop clinical pharmacy skills and expertise within a range of clinical specialties: obstetrics and gynaecology, special care and neonatal intensive care, paediatrics and adolescent health, oncology and haematology, adult intensive care and coronary care and interventional cardiology.
The central dispensary also provides an extended hours service for all our hospitals so the role will require you to be available to work weekdays as well as sharing the evening, weekend and on-call rosters.
The salary for this position is $41.87 to $56.07 per hour plus superannuation and benefits.
