Nurse aged care coordinators play a vital role in delivering person-centered care to our ageing population.
Key Responsibilities:- Coordinate and implement individualized care plans to achieve restorative goals for clients.
- Work collaboratively with healthcare professionals to promote wellness, reablement, and independence.
- Conduct regular assessments and reviews to monitor client progress and adjust care plans accordingly.
- Provide education and support to clients and their families on various aspects of aged care, empowering them to make informed decisions about their health and well-being.
