About the Role

The Child Health Outreach Program (CHOP) Team Leader is responsible for providing management and coordination of the CHOP, ensuring cultural integrity and high standards of clinical care and case management.

This position has a strong focus on outreach work with clinical, liaison and administrative responsibilities. It requires a leader who can manage a multidisciplinary team effectively.

Requirements

To be considered for this position, you will require:

  • Current registration as a Registered Nurse or Aboriginal Health Practitioner with AHPRA
  • At least five years clinical experience
  • Demonstrated ability to lead and manage a multidisciplinary team
  • Experience in community-based child health nursing, case management and chronic disease management
What We Offer

We offer a total effective package of $142,560 - $154,051 per annum, which includes base salary, district allowance, superannuation, leave loading and estimated tax saving from salary packaging options.
