The Care Support Specialist will assist clients navigate their Transitional Aged Care Program journey with compassion and expertise. This role involves supporting clients and their families through short-term, specialised care and support services to help regain functional independence at home after a hospital stay.

Responsibilities:
  • Home visiting clients who have been referred for Transitional Aged Care Program case management and services at home after a hospital stay

Requirements:
