Exercise Physiologist

The In Home Care (IHC) Exercise Physiologist is a key member of our multidisciplinary team, responsible for delivering high-quality, best-practice care to consumers in a community setting and Baptcare's retirement living. As a Clinical Rehabilitation Specialist, you will work collaboratively with various funding bodies, including Home Care Packages, Short Term Restorative Care, CHSP, and privately funded clients.

• Develop and implement person-centred exercise programs for clients with chronic and complex conditions, working within a multidisciplinary team.
• Identify need for additional support services with the customer in care plans, making appropriate internal and external referrals.
• Minimise infection-related risks through implementing standard transmission-based precautions to prevent and control infection, ensuring compliance with all Work, Health, and Safety responsibilities.
• Report customer risks to management, including inappropriate behaviours or actions not aligned to our values or the Aged Care Quality Standards.
• Ensure scope of practice is compliant with regulatory guidelines and Baptcare policy and procedures.
• Provide follow-up reviews, interventions, and self-management strategies to empower the customer to achieve agreed goals in line with chronic disease and best-practice principles.
• Provide feedback to customers, referrers, and other stakeholders outlining assessment findings, agreed goals, and recommendations.
